The ongoing Ebola outbreak in the Democratic Republic of Congo (DRC) poses a significant challenge to global health authorities, and there are several reasons why containing this outbreak is proving to be difficult. Firstly, late detection and insecurity in the region are major factors. The DRC's familiarity with Ebola outbreaks, coupled with ongoing instability and conflict, particularly around the town of Goma, has led to a delay in identifying and isolating infected individuals. This delay is crucial as Ebola is highly contagious, and early detection is essential to control its spread. The time between infection and diagnosis, known as the detection lag, can vary between one and 32 days, and this lag contributed to the rapid spread of Ebola during the 2013-2016 outbreak in Sierra Leone. Secondly, misdiagnosis is another critical issue. Early symptoms of Ebola can be subtle and similar to those of malaria or typhoid, leading to potential misdiagnosis. During the early days of the Ebola outbreak in Sierra Leone, nurses misdiagnosed the disease as Lassa fever, resulting in their deaths. Additionally, the Bundibugyo virus, which is causing the current outbreak, is relatively new and less researched compared to the Zaire Ebola virus, making diagnosis and treatment more challenging. Cultural factors also play a significant role in the spread of Ebola. Ritual burials, a common practice in many African countries, including Sierra Leone and the DRC, can elevate the risk of infection. The ceremonial preparation of the body and communal grieving can lead to the transmission of the virus, as evidenced by the tragic death of scores of people during the 2013-2016 outbreak in Sierra Leone. Lastly, the shortage of global health funds and the dissolution of the United States Agency for International Development (USAID) have impacted the effectiveness of public health activities worldwide. The DRC, despite its experience in tackling Ebola outbreaks, faces challenges due to a lack of experts and logistics on site, leading to an extended delay in managing the outbreak. To address these issues, a rapid, multi-tiered response is required, focusing on rapid case detection, multinational support, swift collaborative surveillance, and community engagement. Community engagement is crucial to raise awareness and sensitisation, especially in regions with unsafe burial practices. Enforcing public health laws and educating communities about the risks of Ebola transmission are essential steps to prevent further spread and protect public health.
